Clearance Information to submit fall sports forms

Attention ALL Student-Athletes trying out for a fall sport! The forms you need in order to clear:

All of the forms that are due every fall season, regardless of your playing experience:
1) Student-Parent Athletics Participation Contract & Permission Form (all parts completed)
2) Medical Card – both sides/all parts filled out and signed
3) Pre-Participation Head Injury/Concussion Report Form
4) Concussion Awareness and Sudden Cardiac Arrest Awareness Acknowledgement

Due once a calendar year:
5) The physical form (SR-8) is valid for one year and must be signed and dated my a doctor or nurse practitioner. Only the Medical Clearance form SR-8/Physical Evaluation should be submitted.   The date of the doctor’s signature is kept on file. The nurse’s office keeps these forms.

This form is only due to the athletics office if you need to complete a new baseline test. The baseline test is valid for two calendar years.
6) Impact Testing Consent Form. Note: You may have completed this test during past tryouts. It is valid for two years.  If you are not sure, fill out this form anyway.

If you are trying out for football, you must turn in this form:
7) Proof of Medical Insurance

Bring all completed forms to any one of the Clearance Events which are:

Monday August 7th from 4-6pm, Tuesday August 8th from 2 – 5pm, Thursday August 10th from 2 – 5pm and Friday August 11th from 12 – 2pm. All clearance will take place in room 101. These are the ONLY opportunities to turn in forms. Latecomers will need to contact the Athletic Director to set up their clearance (

A Blair Athletics staff member will review your forms when you bring them in.  If your forms are complete, you will receive a signed clearance form.  This form goes to the coach and allows you to tryout/participate in that sport.
